Buccal Fat Removal vs Face Lipo: Which Is Better in Malaysia?

One of the more interesting consultations I have almost every week goes something like this:
“Doctor, I want a smaller face… but I don’t know whether I need buccal fat removal or face lipo.”
And honestly, I don’t blame patients for being confused. Online, both treatments are often marketed under the same idea — “face slimming” — but clinically, they are quite different.
The first thing I usually do is take a step back and look at the face as a whole, not just the “fat”.
Because not all fullness in the face comes from the same layer.
Some people have volume sitting deeper in the cheek — what we call the buccal fat pad. Others have more superficial fat along the lower face and jawline. And these two behave very differently.
Understanding the Difference (This is where most people get confused)

Buccal fat removal is actually quite specific. It targets a deep fat compartment that gives that soft, rounded cheek appearance. When removed, the effect is more of a hollowing — you get that sculpted, slightly sunken cheek look.
For the right patient, it can look very elegant.
But for the wrong patient, especially in Asian faces where volume loss happens with age, it can make someone look older faster than expected. That’s something I’m always cautious about.

Face Lipo (Laser Lipolysis – e.g. Accusculpt)
Face lipo, on the other hand, works closer to the surface. It’s less about hollowing and more about defining.
When I perform laser lipolysis for the face, I’m usually targeting areas like the lower cheek, jawline, or under the chin. The goal is not to remove all the fat, but to reshape how the light hits the face — sharper angles, cleaner contour.
Interestingly, many patients who initially ask about buccal fat removal don’t actually need it. What they’re seeing in the mirror is often lower face heaviness, not mid-cheek volume.
And if you remove the wrong fat, you don’t get the result you were hoping for.
So… Which One Is Better?
Honestly — neither is “better”.
They solve different problems.
Here’s how I explain it to patients:
If your face looks full in the middle (cheek area) when you smile → buccal fat
If your face looks heavy in the lower face/jawline even withour smiling → face lipo
The Biggest Mistake I See
Some patients request buccal fat removal when their issue is actually lower face fat.
Result?👉 Face becomes hollow, but still no jawline definition
That’s why proper assessment matters.
Can You Combine Both?
Yes — but not everyone needs both.
In fact, in Malaysia, most patients benefit more from:👉 Face lipo + skin tightening (instead of buccal removal)
Because:
Asian faces tend to age with volume loss later
Over-removal early can lead to premature aging look
My Clinical Advice
If you’re unsure:
Start with less aggressive, contouring treatments
Only consider buccal fat removal if truly indicated
